MEDICATIONS

 If your child must have medication of any type, including over-the-counter medicine, given during school hours, you have a few choices:       

· You may come to the school and give it to your child at the appropriate time.

· You may get a medication form and have your doctor indicate on the form the drug, dose, and time to be given.  Make sure he signs the form.  We also need the pharmacy labeled bottle containing the medication and the instructions regarding how it is to be given.

· You may discuss with your doctor an alternative schedule of medication so it can be given outside of school hours.

MEDICAMENTO

Si su hijo debe recibir medicamentos de cualquier tipo, incluidos los medicamentos de venta libre, administrados durante el horario escolar, tiene algunas opciones:

· Puede venir a la escuela y dárselo a su hijo en el momento adecuado.

· Puede obtener un formulario de medicamentos y hacer que su médico le indique en el formulario el medicamento, la dosis y la hora a administrar.

 · Asegúrese de que firme el formulario. También necesitamos el frasco con la etiqueta de la farmacia que contiene el medicamento y las instrucciones sobre cómo administrarlo. ·

· Puede discutir con su médico un programa alternativo de medicamentos para que pueda administrarse fuera del horario escolar.
